12 noon please feel free to pop in to have a chat with john on the above days/times to see if he can help you to tap into the hundreds of local support services in our area and even help/inspire you to make positive changes to your health and well being. gp care wakefield launched gp care wakefield is an extended hours service owned by all wakefield gp practices to cover the entire population of the wakefield district. it operates from 6-10pm monday to friday, and 9-3pm saturdays, sundays and bank holidays. just phone the normal surgery telephone and the call will be automatically diverted to a triage centre where your clinical need will be assessed and if deemed appropriate you will be offered a gp or anp appointment at the trinity medical centre in wakefield or pontefract general infirmary. there needs to be shared consent in place for the service to be able to provide safe care therefore please ensure you give explicit record sharing consent when accepting an appointment with the service. patients who prefer not to give shared consent cannot use this service. the service is not available for routine matters such as repeat prescriptions. outside of the above hours please telephone 111 or try booking on our 24hr appointment system if your need is not urgent. routine care by hca/nurse in addition to the above, during 6pm -
